Office Policies - Please Read

I am available to see patients by appointment only, no drop-in office visits please.
Rates and Forms of Payment:
First Office Visits are one and a half hours (90 minutes) - $240.00.
Return Office Visits are forty-five (45) minutes - $135.00.
Payment in full is due at the time of service. Supplements purchased from the clinic will be in addition to the charge for the office visit. Accepted forms of payment are checks and credit cards (Visa, MasterCard, Discover and American Express). I also accept payment directly from your Flexible Spending Account (FSA) or your Health Savings Account (HSA). I do not bill insurance companies directly, but I do provide you with a copy of your complete invoice so that you may submit it for reimbursement.
Please note that the First Office Visit fee and duration of that visit apply to all new patients AND previous patients who have not seen me in over 2 years.
New Patient Forms/Intake: When you schedule for your First Office Visit, you will receive instructions on how to log into the Electronic Health Records (EHR) Patient Portal for my clinic. The EHR portal will allow you to easily fill out your New Patient Intake Forms and other forms prior to your appointment. It is important to complete your paperwork ahead of your visit so that I am able to review your information before our first appointment together.
Returning Patient Intake: Once you are established within the patient portal, when you go to schedule your Return Office Visits, you are able to provide information within the patient portal regarding your health needs at that time.
Please call, email or fill out the online form by clicking here, to let me know you're interested in scheduling an office visit. New Patient Office Visits are booked for one and a half hours (90 minutes), Return Office Visits for forty-five (45) minutes. Once you choose an appointment time and day that works for you, you are not definitively booked until you receive a phone call or an email confirmation from me.
There is a two (2) business-day cancellation policy. If you are unable to make it to your appointment, I ask that you provide at least two business days' notice so that I may offer that appointment time to someone else. If you notify me of your cancellation in less than two business days, or if you miss your appointment, a cancellation charge will apply. For a First Office Visit the cancellation charge is $105.00, for a Return Office Visit, the cancellation charge is $75.00.
I realize that an emergency may occur and you may not be able to notify me on time. I will discuss that situation with you if it happens.
The online email forms available within this website go to my clinic email. I respond directly to these email messages and they are for brief inquiries only, to obtain more information about my practice. I do not provide health advice via email for new or established patients.
As a courtesy, for established patients, you may send a secure message to me within the HIPAA-complaint patient portal. Through this system, patients may send short emails for non-urgent, medically related questions. If the issue you are emailing me about takes me 5 minutes or less, there will be no charge. If it takes me longer than 5 minutes to address, you will be charged accordingly, for the time I spend on email addressing your concerns.
I do not provide health advice via email or messaging, but I can advise you if there is a minor change to be made to your treatment plan, or if you are needing clarification on an aspect of your treatment plan.
If the issue appears to be more complex or is regarding a new health issue, I will recommend that you schedule an office visit.
I address phone messages during regular office hours, Tuesday - Thursday. I will return calls when I am between office visits or at the end of the day. Please understand that I manage phone calls directly, therefore when I am with patients I am unable to answer the phone or address phone calls.
If your phone call is in regards to a more complex health issue, I will recommend you schedule an office visit.
Your lab results belong to you. I provide copies of these results to you after we have discussed the results in an office visit. Lab results are also available to you via the Patient Portal; once you have established yourself as a patient, you can access your lab results there.
SUPPLEMENTs and Supplement REFILLS:
I carry a certain number of supplements at the office. Depending on my recommendations, there may be supplements that I order for you, if I don't currently have them in stock. These can be shipped directly to your home.
For supplement refills, please send me a message through the Patient Portal and make this request at least 7-10 days before you run out of your supplements. With the refill process, you can either arrange to pick-up your supplements during my office hours, or they can be shipped to you.
When a medication is prescribed for long-term use, I typically provide 1-3 months worth of refills. When you are in need of a prescription refill, please contact your pharmacy, at least a week before you are out of your medication, to process that refill.
If you are low on your medication, and out of refills, please contact the office to schedule a return office visit so that I can assess your current health status and determine how best to move forward in terms of your medication. Please plan in advance for this process, before you run out of your medication.
Emergency and Urgent Care Needs:
I work with patients between the hours of 10.00am-6.00pm, Tuesday - Thursday, by appointment only. If you experience a health emergency or urgent care issue, please call 9-1-1 or visit your closest urgent care facility or emergency department.
